All patients who are Covid 19 positive must complete the CDC recommended 5-day isolation period before being scheduled to come into the office for lab or office services and wear a mask in the office if they have recently had Covid or are suffering from other respiratory symptoms. Masking is voluntary currently, however highly recommended for those vulnerable to respiratory illness, and those in high-risk populations such as immunosuppressed and dialysis patients.

Prescriptions & Refills

Please bring an updated medication list to every appointment. You may request refills through our patient portal. Prescriptions will be filled as generic, unless otherwise specified. Pease specify if you need a 30-day or a 90-day supply, and let us know to which pharmacy you would like the refills sent. We process refills for prescriptions that we have prescribed for you from this office. We do not e-prescribe for controlled substances. Thank you

Our practice includes 5 American Board of Internal Medicine (ABIM) certified Nephrologists, 4 Nurse Practitioners, and 3 Physician Assistants who work collaboratively to meet the needs of our patients.
